Explanation

The correct answer according to the source is the Ministry of Youth Affairs and Sports, Government of India.
The book states that the Rajiv Gandhi Khel Ratna Award is accorded annually by this ministry.
It further describes the award as the highest sporting honour of the Republic of India and says that it was first awarded to sports personalities in 1991-92.
According to the source, up to 2020 a total of 43 persons had received this prize.
Therefore the source-supported revision chain is: Rajiv Gandhi Khel Ratna Award — Ministry of Youth Affairs and Sports, Government of India — annual award — described by the source as India's highest sporting honour — first awarded in 1991-92 — 43 recipients till 2020 according to the book.
The recipient count is explicitly a dated figure from the source and should not be interpreted as a current total.
The source uses the historical name 'Rajiv Gandhi Khel Ratna Award', which has been preserved rather than replaced with later terminology.
Therefore option (d) is correct according to the source.

Ministry of Youth Affairs and Sports, Government of India Rajiv Gandhi Khel Ratna Award — Ministry of Youth Affairs and Sports — Annual Award — स्रोत के अनुसार भारत का Highest Sporting Honour — First Awarded in 1991-92 — 2020 तक 43 recipients।
