Why is Patient Safety in News?
The National Conference on Patient Safety 2026 was successfully conducted at the All India Institute of Medical Sciences, Nagpur. The national-level conference brought healthcare leaders, clinicians, nursing professionals, hospital administrators, researchers and patient-safety specialists together to discuss ways of reducing preventable harm in healthcare.
The conference was organised by AIIMS Nagpur in collaboration with the National Patient Safety Secretariat under the Directorate General of Health Services, Ministry of Health and Family Welfare.

What is Patient Safety?
Patient safety refers to organised efforts to prevent avoidable harm to patients during healthcare delivery.
Healthcare can save lives, but mistakes in medication, surgery, diagnosis, infection control, communication or blood transfusion can cause preventable injury. Patient-safety systems attempt to identify these risks before they harm patients.

What are Non-Communicable Diseases?
Non-communicable diseases, commonly abbreviated as NCDs, are diseases that are generally not transmitted directly from one person to another.
Major NCD groups include cardiovascular diseases, cancers, diabetes and chronic respiratory diseases.
Why NCD Patient Safety Matters
Many people with diabetes, hypertension, heart disease or other chronic conditions take multiple medicines for long periods. They may also receive treatment from different specialists.
This creates risks such as incorrect dosage, harmful drug interactions, duplication of medicines and communication gaps between healthcare providers.
Medication Safety
Medication errors can occur during prescribing, dispensing or administration of medicines. Safe systems therefore require correct identification of the patient, medicine, dose, route and timing.
Electronic records, prescription review, pharmacist involvement and clear communication can help reduce errors.
Surgical Safety
Surgical safety involves ensuring that the correct procedure is performed on the correct patient at the correct site, while also preventing infection and managing anaesthesia-related risks.
Checklists are widely used as a simple but effective tool for improving surgical safety.
Healthcare-Associated Infections
Patients can sometimes acquire infections while receiving medical treatment. Infection prevention therefore includes hand hygiene, sterilisation, safe injection practices, environmental cleanliness and appropriate use of antimicrobial medicines.
Antimicrobial Resistance
Patient safety is also connected with antimicrobial resistance. Unnecessary or inappropriate use of antibiotics can encourage microorganisms to develop resistance.
Antimicrobial stewardship seeks to ensure that antibiotics and other antimicrobial medicines are used only when needed and in the correct manner.
Role of Patients and Families
Modern patient-safety approaches increasingly recognise patients and families as active partners rather than passive recipients of care.
Patients should be encouraged to understand their medicines, ask questions, report allergies and communicate previous medical conditions.

रोगी सुरक्षा का अर्थ स्वास्थ्य सेवा प्राप्त करते समय रोगी को होने वाली ऐसी हानि को रोकना है जिसे उचित व्यवस्था और सावधानी से टाला जा सकता है।
दवा, शल्य चिकित्सा, निदान, संक्रमण नियंत्रण, संचार अथवा रक्त आधान से जुड़ी गलतियां रोगी को नुकसान पहुंचा सकती हैं। रोगी-सुरक्षा व्यवस्था इन जोखिमों को पहले पहचानने और कम करने का प्रयास करती है।

गैर-संचारी रोग क्या हैं?
Non-Communicable Diseases अर्थात NCDs ऐसे रोग हैं जो सामान्यतः एक व्यक्ति से दूसरे व्यक्ति में सीधे नहीं फैलते।
हृदय और रक्तवाहिका संबंधी रोग, कैंसर, मधुमेह तथा दीर्घकालीन श्वसन रोग इसके प्रमुख उदाहरण हैं।
NCD रोगियों की सुरक्षा क्यों महत्वपूर्ण है?
मधुमेह, उच्च रक्तचाप और हृदय रोग से पीड़ित अनेक रोगियों को लंबे समय तक कई प्रकार की दवाएं लेनी पड़ती हैं। कभी-कभी अलग-अलग विशेषज्ञ उनका उपचार करते हैं।
इससे गलत मात्रा में दवा, दवाओं के बीच हानिकारक प्रतिक्रिया, एक ही दवा का दोहराव और चिकित्सकों के बीच जानकारी की कमी जैसी समस्याएं हो सकती हैं।
दवा सुरक्षा
दवा से संबंधित गलती prescription लिखने, दवा देने अथवा रोगी द्वारा दवा लेने के दौरान हो सकती है। सुरक्षित व्यवस्था में सही रोगी, सही दवा, सही मात्रा, सही माध्यम और सही समय सुनिश्चित करना आवश्यक है।
शल्य चिकित्सा सुरक्षा
शल्य चिकित्सा में यह सुनिश्चित किया जाना चाहिए कि सही रोगी पर सही स्थान पर सही प्रक्रिया की जाए। इसके साथ संक्रमण और anaesthesia से संबंधित जोखिमों को भी नियंत्रित करना आवश्यक है।
अस्पताल से संबंधित संक्रमण
कभी-कभी रोगी उपचार प्राप्त करते समय संक्रमण का शिकार हो सकता है। इसलिए हाथों की स्वच्छता, उपकरणों का उचित sterilisation, सुरक्षित injection, स्वच्छ वातावरण और antimicrobial medicines का उचित उपयोग महत्वपूर्ण है।
Antimicrobial Resistance
Antimicrobial resistance भी रोगी सुरक्षा से जुड़ी चुनौती है। Antibiotics का अनावश्यक अथवा गलत उपयोग microorganisms में resistance बढ़ा सकता है।
Antimicrobial stewardship का उद्देश्य इन दवाओं का आवश्यक स्थिति में और सही तरीके से उपयोग सुनिश्चित करना है।
रोगी और परिवार की भूमिका
आधुनिक रोगी-सुरक्षा व्यवस्था में रोगियों और उनके परिवारों को उपचार प्रक्रिया का सक्रिय भागीदार माना जाता है।
रोगियों को अपनी दवाओं की जानकारी रखने, allergies बताने, प्रश्न पूछने और पुराने रोगों की सही जानकारी चिकित्सक को देने के लिए प्रोत्साहित किया जाना चाहिए।
